Every Ukiah Unified student needs to complete an Educational Benefit Form this year. Completing this form allows us to qualify for funding that supports critical programs such as music, art, athletic programs, technology, counselors, reduced class size, and so much more. If you need help completing the form, please contact your school’s Family Community Liaison.

Eagle Peak Families if you wish to sign your student up for ASES here at Eagle Peak, Please fill out this online application. If your student is in 5th Grade, please continue to fill out this form as there is an option to select 5th Grade.
